Your First Visit
When you arrive for your first appointment, you will be required to complete registration and medical history forms. These forms can also be accessed on this website and completed prior to your appointment.
If you have been given a referral slip and/or x-rays by your dentist or physician, please remember to bring them with you along with any dental and medical insurance cards. This will save time and allow us to help process your claim in a timely manner.
During your appointment, a thorough review of your medical history will be taken, including the medications you are currently taking. If necessary, please provide a list, including any over the counter medications, vitamins, supplements and/or diet pills.
Your diagnosis and treatment options will be discussed in detail.
If treatment is indicated, informed consent for each procedure will be discussed, written informed consent forms reviewed and, if applicable, procedure specific informed consent videos will be viewed. Many of these on also available on this web site.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian for all appointments.
You will receive pre-operative instructions, necessary prescriptions and then be scheduled for your procedure.
Please alert the office if you have a change in your medical condition prior to your scheduled surgery (i.e. cold, fever, infection, etc.)
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office or you can bring them with you on your first visit. Utilizing the latest digital radiographic equipment, any additional images can be taken at our facility.
